New Insurance and Identification Requirements
Insurance companies are implementing stricter requirements for documentation. To avoid delays or complications during your visit, please bring the following:

All of your insurance cards. This includes primary and secondary insurance if applicable.

Your driver’s license or a government-issued ID.
Having these documents readily available will help us verify your information and ensure a seamless check-in process.
Updated Medication List
In addition to your insurance and identification, please bring a current list of all medications you’re taking. This information is crucial for providing the best possible care and avoiding any potential interactions or complications.
Cancellation Policy Reminder
We understand that life happens and schedules change. However, starting in 2025, we require at least 24 hours’ notice for appointment cancellations.
Appointments canceled with less than 24 hours’ notice will incur a $50 missed appointment fee. This policy helps us accommodate other patients who may need care and ensures our schedule runs efficiently.
Copayments Due at Time of Service
To streamline our billing process and align with insurance requirements, all copayments will now be due at the time of service. We will no longer bill copays at a later date, so please plan accordingly when preparing for your appointment.
